Researchers Uncover Biological Rationale For Why Intensive Lupus Treatment Works
Researchers at UT Southwestern Medical Center have uncovered the biological rationale for why large doses of corticosteroids given repeatedly over several weeks may help individuals with lupus, a chronic inflammatory disease that affects more than 1 million people in the U.S. Unlike the anabolic steroids athletes sometimes use illegally to bulk up muscle, corticosteroids are routinely used to treat inflammation in lupus patients. The drugs, however, can cause undesirable side effects including weight gain and acne when taken over long periods of time... (Source: Health News from Medical News Today)

Can sebum reduction predict acne outcome?
The objective of this analysis was to explore the correlation of sebum and acne by combining data from studies of various acne treatments that have demonstrated effects on both sebum excretion and acne outcome. Acne measures included total lesion count, inflammatory lesion count and acne severity grade. For each acne measure, data were pooled and analysed at the 3- and 4-month endpoints, when sebum reduction has generally equilibrated and efficacy in acne is approaching the maximum effect for most treatments. A linear model was used to describe the percentage reduction in each acne measure as a function of percentage reduction in sebum excretion. Slope values were similar for the three acne parameters and all were significantly different from zero (P < 0·025), suggesting a significant cor...

Prophylactic low-level light therapy for the treatment of hypertrophic scars and keloids: A case series
Hypertrophic and keloid scars result from alterations in the wound healing process. Treating abnormal scars remains an important challenge. The aim of this case series was to investigate the effectiveness of near infrared (NIR) light emitting diode (LED) treatment as a prophylactic method to alter the wound healing process in order to avoid or attenuate the formation of hypertrophic scars or keloids.Three patients (age 27-57) of phototypes I-III with hypertrophic scars or keloids due to acne or surgery participated in this case series. Following scar revision by surgery or CO2 laser ablation on bilateral areas, one scar was treated daily by the patient at home with non-thermal, non-ablative NIR LED (805 nm at 30 mW/cm2) for 30 days. Efficacy assessments, conducted up to a year post-treatme...

ACNE: a summarization method to estimate allele-specific copy numbers for Affymetrix SNP arrays
Motivation: Current algorithms for estimating DNA copy numbers (CNs) borrow concepts from gene expression analysis methods. However, single nucleotide polymorphism (SNP) arrays have special characteristics that, if taken into account, can improve the overall performance. For example, cross hybridization between alleles occurs in SNP probe pairs. In addition, most of the current CN methods are focused on total CNs, while it has been shown that allele-specific CNs are of paramount importance for some studies. Therefore, we have developed a summarization method that estimates high-quality allele-specific CNs. Results: The proposed method estimates the allele-specific DNA CNs for all Affymetrix SNP arrays dealing directly with the cross hybridization between probes within SNP probesets. This a...

Photodynamic therapy for acne vulgaris: A critical review from basics to clinical practice: Part II. Understanding parameters for acne treatment with photodynamic therapy
Photodynamic therapy requires a photosensitizer, oxygen, and activating light. For acne, pilosebaceous units are “target” structures. Porphyrins are synthesized in vivo from 5-aminolevulinic acid (ALA), particularly in pilosebaceous units. Different photosensitizers and drug delivery methods have been reported for acne treatment. There are a variety of porphyrin precursors with different pharmacokinetic properties. Among them, ALA and methyl-ester of ALA (MAL) are available for possible off-label treatment of acne vulgaris. In addition, various light sources, light dosimetry, drug incubation time, and pre- and posttreatment care also change efficacy and side effects. None of these variables has been optimized for acne treatment, but a number of clinical trials provide helpful guidance...

Photodynamic therapy for acne vulgaris: A critical review from basics to clinical practice: Part I. Acne vulgaris: When and why consider photodynamic therapy?
The first-line treatments for acne vulgaris are conventional topical and/or oral medications. However, many patients have contraindications, only partial response, significant adverse effects, or recurrence. Light-based treatments and photodynamic therapy (PDT) using topical precursors of porphyrins are off-label alternative treatments for acne vulgaris, with their own advantages and disadvantages. To date, there is no consensus on PDT methodology and parameters. An understanding of pathophysiology of acne, light–tissue interactions, and PDT mechanisms of action is helpful whenever PDT is considered as an alternative treatment. In general, blue light alone improves acne because of its antiinflammatory effects. PDT using 5-aminolevulenic acid (ALA) or ALA derivatives induces transient ant...

Problems in the reporting of acne clinical trials: a spot check from the 2009 Annual Evidence Update on Acne Vulgaris
In the course of producing the 2009 NHS Evidence - skin disorders Annual Evidence Update on Acne Vulgaris, 25 randomised controlled trials were examined. From these, at least 12 potentially serious problems of trial reporting were identified. Several trials concluded no effect of a treatment yet they were insufficiently powered to exclude potentially useful benefits. There were examples of duplicate publication and "salami publication", as well as two trials being combined and reported as one. In some cases, an incorrect "within-groups" statistical comparison was made and one trial report omitted original efficacy data and included only P values. Both of the non-inferiority studies examined failed to pre-specify a non-inferiority margin. Trials reported as "double-blind" compared treatment...

Interleukin-1A +4845(G>T) polymorphism is a factor predisposing to acne vulgaris
Acne vulgaris is a common chronic inflammatory skin disease of multifactorial origin. The aim of this study was to clarify whether known polymorphisms of the interleukin-1A (IL1A) and IL1RN genes play a role in the pathogenesis of acne vulgaris. A positive association was found between the minor T allele of the IL1A +4845(G>T) single nucleotide polymorphism (SNP) and acne, whereas no association was found with respect to any alleles of the variable number of tandem repeats (VNTR) polymorphism of the IL1RN gene. The severity of inflammatory acne symptoms correlated with the percentage of individuals carrying the homozygote T/T genotype. These results may help to elucidate the molecular events leading to the development of acne. (Source: Tissue Antigens)

Treatment of Facial Postinflammatory Hyperpigmentation with Facial Acne in Asian Patients Using a Q-Switched Neodymium-Doped Yttrium Aluminum Garnet Laser
Postinflammatory hyperpigmentation (PIH) due to facial acne commonly occurs in Fitzpatrick skin types IV to VI and can have a considerable effect on quality of life. The sequelae in terms of pigmentation are often the main reason for consulting a dermatologist and take precedence over the acne itself. The purpose of this study was to evaluate a new combined photoacoustic[ndash]photothermal low-fluence Q-switched neodymium-doped yttrium aluminum garnet (QSNY) laser in the treatment for facial acne and PIH. Twenty individuals underwent five weekly full-facial treatments using a QSNY laser at 1,064 nm. Another 20 individuals underwent five weekly facial acne treatments by extraction of comedones, and intralesional injections into severely inflamed papules were performed at each treatment sess...

Evaluation of estrogen receptor Beta binding of pruni cortex and its constituents.
In this study, we found that water extracts of Pruni Cortex, not Quercus Cortex showed a binding effect on estrogen receptor beta (ERbeta). Thus, their chemical analysis was carried out by GC-MS and found that five unique constituents (i.e., sakuranetin, naringenin, genistein, genkwanin and arctigenin) were detected in Pruni Cortex. The ERbeta binding capacity of these constituents was examined using 70 ng/ml 17beta-estradiol, as the positive control with 100% ERbeta binding. Among them, genistein (60% at 10 ng/ml) showed the strongest binding capacity than naringenin (60% at 1000 ng/ml) and sakuranetin (40% at 1000 ng/ml). These results suggested that Pruni Cortex in JHT could play an important role in treatment of acne. In addition, those of Pruni Cortex from different harvest places wer...

Healing fats of the skin: the structural and immunologic roles of the ω-6 and ω-3 fatty acids
Abstract: Linoleic acid (18:2ω6) and α-linolenic acid (18:3ω3) represent the parent fats of the two main classes of polyunsaturated fatty acids: the ω-6 (n-6) and the ω-3 (n-3) fatty acids, respectively. Linoleic acid and α-linolenic acid both give rise to other long-chain fatty acid derivatives, including γ-linolenic acid and arachidonic acid (ω-6 fatty acids) and docosahexaenoic acid and eicosapentaenoic acid (ω-3 fatty acids). These fatty acids are showing promise as safe adjunctive treatments for many skin disorders, including atopic dermatitis, psoriasis, acne vulgaris, systemic lupus erythematosus, nonmelanoma skin cancer, and melanoma. Their roles are diverse and include maintenance of the stratum corneum permeability barrier, maturation and differentiation of the stratum c...

Detection of a new erm(X)-mediated antibiotic resistance in Egyptian cutaneous propionibacteria.
Authors: El-Mahdy TS, Abdalla S, El-Domany R, Mohamed MS, Ross JI, Snelling AM A total of 107 antibiotic-resistant propionibacteria were isolated from the face of 102 Egyptian acne patients, dermatology staff and controls. Erythromycin-clindamycin-resistant propionibacteria were chosen to detect erm(X) gene and it was detected in 29 of 107 (27%) strains. However, just 7 strains had IS1249I, 3 of them had also Tn5432. The erm(X) gene which is not carried on Tn5432 confers inducible resistance to telithromycin by erythromycin or clindamycin. The DNA sequences of the PCR amplification products of this new erm(X)-mediated antibiotic resistance showed >99% identity to the erm(X) gene isolated from a Corynebacterium jeikeium. Southern blotting analysis of the erm(X)-specific probe shows t...

Chloracne: First manifestation of dioxin toxicity
3 out of 5 stars Chloracne: Still Cause for Concern. Passasrini B et al.  Dermatology [Published online June 2 2010 ahead of print] Abstract When Viktor Yushchenko (picture) — a candidate for President of the Ukraine — was poisoned with dioxin in 2004, the image of his ravaged face was an unforgettable reminder of the dermatologic effects of this toxicity. Chloracne is a skin condition caused by exposure to halogenated cyclic hydrocarbons. Features include: • presence of comedones (blackheads) and yellowish cysts on the cheeks, trunk, and other areas not often involved in acne vulgaris (for example, the genital region) • grayish discoloration of involved skin • prominent involvement lateral to the eyes and behind the ears • relative sparing of nose Other manifestations...

Loss of NPC1 function in a patient with a co-inherited novel insulin receptor mutation does not grossly modify the severity of the associated insulin resistance
Abstract  In Npc1 null mice, a model for Niemann Pick Disease Type C1, it has been reported that hepatocyte insulin receptor function is significantly impaired, consistent with growing evidence that membrane fluidity and microdomain structure have an important role in insulin signal transduction. However, whether insulin receptor function is also compromised in human Niemann Pick disease Type C1 is unclear. We now report a girl who developed progressive dementia, ataxia and opthalmoplegia from 9 years old, followed by severe acanthosis nigricans, hirsutism and acne at 11 years old. She was diagnosed with Niemann Pick Disease type C1 (OMIM#257220) based on positive filipin staining and reduced cholesterol-esterifying activity in dermal fibroblasts, and homozygosity ...

Azelaic acid modulates the inflammatory response in normal human keratinocytes through PPARγ activation
Please cite this paper as: Azelaic acid modulates the inflammatory response in normal human keratinocytes through PPAR[gamma] activation. Experimental Dermatology 2010.Abstract: Azelaic acid (AzA), a nine-carbon dicarboxylic acid, is an agent for the topical treatment of acne. It has also been shown to be effective in rosacea; however, the mechanism of action has not been clarified. Because inflammation is a common feature of both conditions, we investigated the effects of azelaic acid on the inflammatory response of normal human keratinocytes to ultraviolet B light, which is a photosensitizer agent in rosacea. AzA, at 20 mm, a concentration achievable following topical application of a 15% gel, suppresses ultraviolet B light-induced interleukins-1[beta], -6 and tumor necrosis factor-[alph...

Innovative Uses for Zinc in Dermatology
Severe zinc deficiency states, such as acrodermatitis enteropathica, are associated with a variety of skin manifestations, such as perioral, acral, and perineal dermatitis. These syndromes can be reversed with systemic zinc repletion. In addition to skin pathologies that are clearly zinc-dependent, many dermatologic conditions (eg, dandruff, acne, and diaper rash) have been associated and treated with zinc. Success rates for treatment with zinc vary greatly depending on the disease, mode of administration, and precise zinc preparation used. With the exception of systemic zinc deficiency states, there is little evidence that convincingly demonstrates the efficacy of zinc as a reliable first-line treatment for most dermatologic conditions. However, zinc may be considered as an adjunctive tre...

A community-acquired methicillin-resistant Staphylococcus aureus liver abscess.
We report such a case in a 25-year-old man who presented with an intermittent fever. He had a history of prolonged antibiotic use for acne and skin abscesses. The liver abscess was successfully treated with percutaneous drainage and a prolonged course of linezolid. To our knowledge, this is the first reported case of a community-acquired methicillin-resistant Staphylococcus aureus liver abscess in Hong Kong, demonstrating the increasing threat posed by this multidrug-resistant organism. This case also suggests that a different epidemiology and route of infection may apply to community-acquired methicillin-resistant Staphylococcus aureus liver abscesses in contrast to the more common pyogenic liver abscesses. Can Sebum Reduction Predict Acne Outcome?
The objective of this analysis was to explore the correlation of sebum and acne by combining data from studies of various acne treatments that have demonstrated effects on both sebum excretion and acne outcome. Acne measures included Total Lesion Count, Inflammatory Lesion Count, and Acne Severity Grade. For each acne measure, data were pooled and analyzed at the 3 and 4 month endpoints, when sebum reduction has generally equilibrated and efficacy in acne is approaching the maximum effect for most treatments. A linear model was used to describe the percent reduction in each acne measure as a function of percent reduction in sebum excretion. Slope values were similar for the three acne parameters and all were significantly different from zero (p<0.025), suggesting a significant correlati...

Heal yourself in 15 days by listening to your body's cries for help
(NaturalNews) Continuing with our 15-day self-healing series, today we're going to shift focus inward. This chapter on self-healing is about learning how to listen to your body and understand what it's really trying to tell you.In medicine, the word "symptoms" is thrown around as if it were something bad. Doctors talk about the "symptoms of disease" and then they prescribe chemicals to suppress those symptoms. If you have pain, they prescribe painkillers. If you have high blood pressure, they prescribe chemicals to artificially lower blood pressure. This practice of detecting and then suppressing symptoms is called "mainstream medicine."But what if that entire approach were wrong? What if symptoms were gifts rather than curses? What if the messages they were sending actually contained impor...

I Need to Review the Periodic Fevers
Discussion Fevers that are perceived by patients, families and health care providers as not following a “normal” pattern bother everyone. Patients and families may have different expectations than healthcare providers about the actual temperature measurement, duration of the fever, and other assorted factors such as where the temperature was taken, the instrument used and whether antipyretic medication had been used. Luckily for most patients, fever, even true recurrent fevers, are often self-limited and have good prognoses. Fever patterns include: Recurrent fever Multiple episodes that occur at inconsistent intervals involving multiple organ systems, fever is one component Multiple episodes that occur at inconsistent intervals involving a single organ system Single episode ...

A Selective Androgen Receptor Modulator Enhances Male-Directed Sexual Preference, Proceptive Behavior, and Lordosis Behavior in Sexually Experienced, But Not Sexually Naive, Female Rats.
Authors: Kudwa AE, López FJ, McGivern RF, Handa RJ Androgens influence many aspects of reproductive behavior, including sexual preference of females for males. In oophorectomized women with sexual desire disorder, testosterone patches improve libido, but their use is limited because of adverse side effects. Selective androgen receptor modulators offer an improved safety profile for both sexes: enhancing libido and muscle and bone growth in a manner similar to steroidal androgens but with fewer adverse effects, such as hirsutism, acne, and prostate growth. The current study investigated the action of a novel selective androgen receptor modulator (LGD-3303 [9-chloro-2-ethyl-1-methyl-3-(2,2,2-trifluoroethyl)-3H-pyrrolo-[3,2-f]quinolin-7(6H)-one]) on male-directed sexual preference, p...

TNFα gene polymorphisms in the pathogenesis of acne vulgaris
Abstract  Inflammation plays an important role in acne pathogenesis, and pro-inflammatory cytokines are key factors in these events. Tumor necrosis factor alpha (TNFα) is a central molecule coded by a gene that shows high level of genetic polymorphisms especially in its promoter region. Single nucleotide polymorphisms (SNPs) of the TNFα gene have been shown to be associated with an increased risk to develop chronic inflammatory diseases. In order to find out if known TNFα regulatory SNPs (−1031T>C, −857C>T, −863C>A, −308G>A, −238G>A) have a role in the development of the inflammatory reactions in acne vulgaris, we analyzed our genomic collection in a retrospective case–control study using the PCR–RFLP method, and we compared the resulting geno...

Adolescence is so unfair!
Carole Jahme shines the cold light of evolutionary psychology on readers' problems. This week: ParentsMy parents suckFrom an anonymous male, age 16Dear Carole, I'm sorry to have to say this but my parents suck BIG TIME. They want to run every part of my life – how much time I spend online, who I bring home (no girls!!!), smoking, the kind of clubs I go to, when I get up in the morning etc etc etc. The really sad thing is my Mum even thinks she has the right to criticise the clothes I wear and how I choose to have my hair. They are so completely out of touch and treat me as if I was still a child. If I had the money I'd be out the door like a flash, but I'm stuck with them at least until I finish my A levels and get a job. I feel like a prisoner. Is there any way I can persuade them to ba...
